Emergency Medical Form
This is the form that allows your leaders to have you seen by a doctor in case of an emergency. To go on any over night trip with NBCC you will need this form.

Parental Approval Form
This is the permission slip form for all overnight student ministries activities. You will need this form for any trips you go on with NBCC.
